In most cases these individuals either have been co-recruited by the Diabetes Center or are members of the Diabetes Clinical Faculty with strong ties to the Diabetes Center. Faculty in this group must fulfill one or more of the following criteria:

a. Substantial collaborative or intellectual interaction with one or more core members of the Diabetes Center, evidenced by common publications and/or grants, or other academic interactions

b. Substantial diabetes-related clinical or research activity at UCSF

c. Significant participation in Diabetes Center-releva nt enrichment activities, which would include seminars, retreats, and other relevant events.

**Diabetes Center membership for faculty in Group 2 will be offered on a case-by-case basis.

Appointment Process

The procedure for appointment to the Diabetes Center is as follows:

* A core faculty member of the Diabetes Center must nominate faculty for appointment. The nominator must discuss the proposal with the potential new member and request a formal written request for membership from the nominee. The request should include a completed application; CV (UCSF format); and other support. The Director of the Diabetes Center will review the request for appointment to determine if it is programmatically consistent with the goals of the Diabetes Center and what resources that might be required to support it. Based on the review, the Director will make a formal recommendation to the Diabetes Center core members.
* Final decision about the appointment will be based on affirmative vote of 2/3 of Diabetes Center core members.

Termination of membership

* Member's request
* Decision of the Diabetes Center Director based on one of the following criteria:
o Discontinuance of activities for which the member was appointed
o Actions detrimental to the Diabetes Center

The appointment will coincide with primary faculty appointment and the DC will participate in all faculty promotion packages put forward by primary department including merits, etc.

B) Diabetes Center “Member-at-Large”

Criteria

This membership is used to create an intellectual community. It is inclusive but dissociated from access to resources (space, salary support and administrative support). Access to Diabetes Center activities will be granted. In cases where the activities of the member fulfill Diabetes and Endocrinology Research Center (DERC) grant membership criteria (for scientist) access to DERC cores will be granted.

- Faculty with Diabetes-related research activity.

- Clinician with Diabetes-related activity.

- Clinicians or Faculty with Diabetes-related teaching activities.

- Anyone with an RO1 (or comparable grant) in an area broadly related to diabetes,
endocrinology and metabolic diseases can be a member.

Appointment Process

1) Interested individuals must write a formal request to Diabetes Center Director, with biosketch and justification of criteria fulfillment.

2) Final decision about membership will be based on the approval of the Membership committee (DERC director + 2 Diabetes Center core faculty).

Note: DERC participants from the bay area are de facto members since they already have submitted a biosketch and research description. This will limit confusion between DERC participation and Diabetes Center membership.

Resource Access: DERC Core, seminars, shared equipment with approval of DC Director, P&F studies.

Participation in DC activity: Seminars, retreats as deemed appropriate by retreat Planning Committee, DERC activities, Happy Hour.

Termination of membership:

1. Member's request
2. Decision of the Diabetes Center Director based on one of the following criteria:

a) Discontinuance of activities for which the member was appointed. Membership will be evaluated every 5 years at the time of the DERC grant renewal.
b) Actions detrimental to the Diabetes Center
